Dear Patient,

Please be aware that from the 1st July 2012 Roundwood Surgery will be changing all patients' medications to 28 days supply only. This is to bring all patients medication in line with Nottinghamshire County PCT prescribing recommendations. This is also to prevent wastage of medication for those patients whose medication may be changed on a regular basis by the hospital.

To submit your repeat prescription, simply complete the form below. Alternatively you can order your repeat prescription by ticking the items you require on your re-order form and posting it in the small letter box just inside the surgery waiting room or you can hand it to your local chemist.

It is very helpful if you can give us 48 hours notice for repeat prescriptions - but we do understand that sometimes this is not possible. We will do our very best to get your prescription ready when you need it.
